Camp runs in week long sessions for rising third through sixth graders.

Half-day Sessions (8:30am - Noon):
June 16th - 20th
June 23rd - 27th

Full-day Session (8:30am - 5:00pm):
July 7th - 11th

By registering online for camp, I hereby acknowledge that I am 18 years of age or older and have read and understand the terms of the Permission and Liability Release and the Audio and Video Release, and the COVID-19 release. I represent that I am the parent, legal guardian, or legal custodian of the minor(s) registered hereafter and have the legal authority to enter into this Release and other registration materials on behalf of such minor. I understand the risks involved with camp and agree to follow all COVID policies in place at the time of camp.

PERMISSION AND LIABILITY RELEASE
I give permission for the registered to participate in the Monticello Summer Adventures program to be taught under the auspices of the Thomas Jefferson Foundation. I understand the nature and scope of this program. I understand that it is not a function of the Thomas Jefferson Foundation, its employees, agents, operators, or instructors to guarantee the safety of participants with respect to this program. I understand that each participant has the responsibility to exercise due care while attending this program for the safety of himself/herself and the other participants. I have made known to Monticello all health information regarding my child’s medical conditions necessary for him/her to participate in summer activities. I have provided Monticello with all medications my child needs or may need. To the extent I have withheld any information, or not provided any medication to Monticello, I release, indemnify, and hold harmless Monticello from any liability or responsibility arising out of or caused by such withholding of information and/or medication. In consideration of the participant’s being permitted to enroll in the activity, I hereby release, indemnify, and hold harmless the Thomas Jefferson Foundation, its employees, agents, operators, and instructors from any and all claims, demands, costs, charges, and expenses for harm, injury, or loss which may be sustained by my child as a result of or relating to participation in this program.
I give permission to the Thomas Jefferson Foundation to seek medical treatment for the registered in the case of an emergency.

AUDIO AND VIDEO RELEASE
I hereby consent to and authorize the use and reproduction by Thomas Jefferson Foundation, Inc. and any of its affiliates and related entities (collectively, the “Foundation”) or anyone authorized by the Foundation, of any and all audio and video recordings and photographic images (the “Property”) that are taken during Monticello Summer Adventures for any publicity, promotional or commercial purpose, without compensation. In addition, I grant to the Foundation and anyone authorized by the Foundation the right to alter the Property without restriction and to copyright the same. I understand that the Property may be used in publications, direct-mail, electronic media (including video, broadcast, CD-ROM, DVD, and Internet/WWW), and other forms of distribution. I release the Foundation, its agents, employees and contractors and those authorized by the Foundation to use the Property from all claims and liability for any violation of any personal or proprietary right I may have in connection with the Property, and all such rights are hereby assigned to the Foundation.
